Birth to Five Years

Answer each question "yes" or "no". If two or more questions are answered "no", the child may have delayed communication and would benefit from consultation and / or evaluation by a Speech-Language Pathologist.

  • Babbles ("gaba, baba") using several consonant sounds
  • Imitates changes in pitch
  • Imitates sounds
  • 1-3 words (other than "mama, dada")
  • Uses gestures for giving, showing, pointing
18 months
  • Locates objects kept in a predictable place on request
  • Understands 12+ familiar vocabulary words
  • Follows 3 word directions ("push the truck")
  • Points to several body parts
  • Names items to indicate wants ("cookie")
  • Uses 10 words, usually nouns
  • Uses "jabbering" that sounds like real speech
  • Imitates words, some phrases
  • Uses more than 5 consonant sounds (p, m, h, n, w, b, k, g, d, t)
2 years
  • Listens to songs/stories for 2-3 minutes
  • Follows one part directions containing a preposition ("Put the ball on the chair")
  • Points to 4-5 major body parts
  • Recognizes 5+ pictures in a book
  • Uses 2-3 word phrases and questions
  • Uses 50 words
  • Speech is understandable about 50% of the time
  • Puts a series of 4+ actions together during play (feeding a doll)
3 years
  • Answers yes/no questions
  • Identifies 3+ objects by function ("Which one do we sleep in?")
  • Distinguishes between "big" and "little"
  • Identifies action in pictures ("Show me the girl who is running.")
  • Has 200-500 word vocabulary
  • Speech is understandable 80% of the time
  • Uses 3-4 word sentences without jabbering
  • Asks "who, what, where" questions
  • Uses plurals, possessives, "ing", articles ("a, the"), "is"
4 years
  • Has number concept of 2
  • Recognized 3-4 primary colors
  • Follows 2-3 part directions containing "in, in back of, next to, in front of"
  • Understands comparison words ("bigger")
  • Has approximately 1,000 word vocabulary
  • Speech is understandable 90% of the time (may not be able to say l, r, ch, sh, s, z, v or j)
  • Speaks in 4+ word complex sentences with clauses (and, if, because)
  • Asks why, how and when questions
  • Uses past tense
5 years
  • Understands time concepts (night / day)
  • Has number concept of 3
  • Follows 3 part directions
  • Asks meanings of words
  • Understands description ("Whose hair is curly?")
  • Listens to a story and answers questions about it
  • Has approximately 1500-1600 word vocabulary
  • Speech is understandable 100% of the time, except for later developing sounds (l, r, ch, sh, s, z, v or j)
  • Speaks in 4-8 word complex sentences containing "when, but, so"
  • Can define words in terms of use ("What do you do with a ____?")
  • Answer "why" questions with a reason
  • Tells familiar story using pictures
